Nutty Mother out collecting donations for daughters death while daughter waits in the car


Tampa, FL - Doctor Luis Ycaza's career is based around caring. So when former patient Jean Fontalvo showed up with a desperate story, asking for donations to cover burial costs for her 13-year-old daughter, he was more than ready to help.
Ycaza stepped up to the plate writing a $500 check, but before handing it over, his staff discovered the mom had a long rap sheet. Dr. Ycaza's willingness to help turned into a desire for justice.
His office tipped off cops and asked Fontalvo for a death certificate. Police say she delivered the form, complete with the name of her 13-year-old daughter Shakira typed in.
Police say the certificate was real, but forged. The age of the child on the form said she was 13, but her date of birth and death indicated the child was 7 months old. Investigators are trying to figure out where she obtained the form.
They say that while Fontalvo was inside Ycaza's office trying to collect the money, her 13-year-old daughter was waiting outside, in the car.
Fontalvo is charged with child neglect, scheming to defraud, grand theft, and uttering a forged instrument. Police say Fontalvo told them she was collecting donations for the Cancer Society. MagazineLane.com - GreatSkin - Skin Care Products
